DEADLINE to add or revise regular courses and request coding for special topics courses:
SPRING 2004 — FRIDAY, AUGUST 29, 2003

IMPORTANT: When submitting a course that is cross-listed, submit all requests together.

For regular courses, the primary department is responsible for submitting the requests.
Please do not staple forms.

• Request for approval of add or revision of regular courses
• Request to drop a regular course
• Request for curriculum codes for special topics courses
• Request for first-year seminar course offering
• Request for W code for research independent study course
  DEADLINE to request W for student enrolled in Research Independent Study is the last day of Drop/Add of the semester the student is enrolled in the course.

Macintosh users need to save these documents in Word first before they can open them
