I. Requests for Course Changes/Coding
A. Regular Courses 001-299: Departments must use the on-line Course Request Form when submitting requests to add, drop, or revise courses numbered 001-299 that appear in the Undergraduate Bulletin. (A different form should be used for Special Topics coding, as per below).
DEADLINE:
TO ADD/REVISE REGULAR COURSES:
- For Spring 2008 Schedule: September 4, 2007
- For Summer 2008: September 21, 2007
- For Fall 2008 Schedule: January 16, 2008
- To Affect 2008-2009 Bulletin: October 15, 2007
TO DROP COURSES FROM UNDERGRADUATE BULLETIN:
B. Graduate School Courses: All departments should use the graduate school forms for courses numbered 200-299 that appear ONLY in the Graduate Bulletin and for ALL courses numbered 300 and above.
C. Special Topics Courses: Select the appropriate online form below and submit it to the Committee on Courses, 011 Allen Building, fax 684-8494.

- For Summer 2008: September 21, 2007
- For Fall 2008 Schedule: January 16, 2008
II. First-Year Seminar Courses: Submit the form below to Dean Norman Keul, 011 Allen Building, fax 660-0488.

III. W (Writing) code for a Research Independent Study Course
- W (Writing) Code Request Form for a Research Independent Study Course
DEADLINE: Last Day of Drop/Add of the semester the student is enrolled in the course. The form with all required signatures should be submitted to 011 Allen Building, fax 684-8494.
IV. Request to Add/Revise/Drop Major, Minor, or Certificate Program
The Committee on Curriculum
charged with addressing proposals made by academic units to add, revise, or drop
majors, minors, and certificate programs.
ADDING:
An academic unit seeking to propose a NEW Major,
Minor, or Certificate
Program should use the appropriate checksheet listed below
when submitting the required supporting materials to the committee.
REVISING/DROPPING:
Requests to REVISE or DROP existing majors, minors, and certificate programs
should be made by means of a letter (from the Director of Undergraduate
Studies/Certificate Program Director) to the Committee on Curriculum providing
the rationale for the request. The material should be sent as an e-mail attachment to harrison.williams@duke.edu. If the request is for a REVISION, please attach a copy of both the proposed revised listing for the Undergraduate Bulletin and the current listing there.
